If you are a Residential Treatment Center looking to have your residents begin their care with our providers, please fill out the New Patient intake form below and review the last pages to include a complete medical history, list out any allergies, and a complete current medication list including dosages and frequency to ensure accuracy when your resident establishes with one of our providers. In addition to the new patient form, please fill out the telemedicine consent form and depression screening form. If a resident is seeking to be placed on MAT services, or already on MAT services, please also fill out the Buprenorphine consent form.
